Request access to information

This form is for requests in terms of the Promotion of Access to Information Act 2 of 2000 (PAIA). Anyone may use it — you do not need a Lifestyle Calculator account. If you only want to see, correct or delete your own personal information, use the Your Data Rights page instead, which is handled under POPIA.

Access is not automatic

Access to records is not automatic. A request may be granted, partially granted or refused in accordance with the applicable provisions of PAIA, including the mandatory and discretionary grounds for refusal in Chapter 4 of the Act. We do not disclose another person's personal information, third-party confidential information, privileged information or other legally protected information merely because a request has been submitted.

Never include bank statements, account numbers, card details, PINs or banking login details in this form.

We aim to acknowledge a PAIA request promptly and to decide on it within 30 days of receiving it, as required by PAIA. That period may be extended in the circumstances the Act allows, and we will tell you in writing if that happens. A request fee or access fee may apply as set out in the PAIA Manual. We use the details you give here only to process and record this request.
